This template creates a note directing the reader to the disambiguation page related to the page it is included in, a page whose name is the name of the current page with "(disambiguation)" appended to it. When the disambiguation page is at a different title, that title can be supplied as an argument to the template.
When to use: When a page has an ambiguous title, so that readers who searched for that title might have been looking for a different article, use this template to direct those readers to a disambiguation page that may help them find their desired article. As suggested here, and illustrated in the example above, please link to a redirect that includes the word "(disambiguation)" in the title, if one exists.
